Scores are the authority. Explanations exist to help people understand—not to renegotiate the number.
Primary scores
Primary scores (such as fall-related and hospital-transfer risk) are composites built from matched rules. Severity badges make level obvious at a glance.
Drivers
Under each primary score, contribution drivers show what pushed the result. Use them in care planning conversations and in the Defense Record.
Domain view
Domain breakdowns (for example mobility-related axes) are not always the same as a composite primary score. Read labels carefully; the product keeps them distinct on purpose.
Explanations
When an explanation is present, it must stay consistent with the scores. If something looks wrong, trust the scores and drivers, and escalate to your administrator—not “edit until it feels better.”
